The Brent Children’s Community Nursing Service provides medical support to children and young people with complex medical needs within their homes and in the wider community . The service aims to enable children and young people with an acute or chronic illness or disability to live with their own families, as independently as possible.

This service provides specialist nursing care for children and young people aged 0-18 who are registered with a Brent GP and have complex health needs arising from disability, accident or illness that cannot be met by general or targeted health services.
If children and young people are eligible for continuing care, they receive an individualised care package, usually provided in the child’s own home as well as school.
Children and young people need to be referred by social services, community nurses, or key workers. The referral will be followed up by a full assessment to determine whether they meet the criteria for children's continuing care support.

The Kingswood Centre offer a service to young people with learning disabilities and mental health issues. They offer a multidisciplinary team consisting of nurses, trained support workers, psychologists and a music therapist etc. including an independent advocate. There are three inpatient units set in a therapeutic environment of green space with nature walks, a sensory garden and a fish pond.
The Dynamic Support Register (DSR) is a vital tool to ensure children and young people with learning disabilities and/or autism get the right care at the right time.
