Our online self-assessment is a digital questionnaire that allows you to assess your own care and support needs at a time and place that suits you.
It guides you through several areas and helps you to identify areas where you might need more help. It can lead to a better understanding of individual needs but may not lead to direct eligibility for social care support.
At the end, you can submit your assessment report to our local Adult Social Care team. They will then contact you to find out more about your situation and will consider whether you would beneffit from a full care needs assessment.

If you are 18 or over, live in Brent and have complex needs that affect your daily life, you are entitled to a free care needs assessment. You can also request one on behalf of someone else, like a relative or friend.
